Prepare for your visit

At each visit you must show your insurance card and a photo ID.
Deductibles and co-pays are required upon check in. We accept most major credit cards, debit cards and cash. We do not accept personal checks.
If you have an unpaid balance over 60 days you will be required to pay balance before being seen in our office.
Self-pay patients must pay for services in full at each visit.
Returned checks are subject to a $30 fee to offset the fee charged by our bank.
We reserve the right to contact Lee County District Attorney’s Worthless Check for collection of a return check.
If you request your records there is a copying fee.
If your visit is for something other than a follow up visit, please make sure you tell the receptionist so we may make sure your insurance is filed appropriately. Ex: Wellness visit, DOT, post hospital…
Refills on medications should be received at your office visit. If you need a refill and it is not time for your appointment please call our main number 334/610-2222and touch 3 for our refill line. Leave your request making sure you give patient name, birthdate, phone number, name of medication and pharmacy. Allow 24 hours for this request to be complete then call your pharmacy to make sure it is ready for pick up. If you are needing a narcotic you will need to pick that up at our office.
If you need precerts for your medication there may be an additional fee for this process.
If you no show for your appointment three times you will be discharged from the practice for non-compliance.
The required HIPPA form has to be signed every year.
Any time you have a change to your address, phone or name please notify our office as soon as possible or use the patient portal, Follow My Health (FMH), to update your demographics.
You can also use the patient portal (FMH) to request a refill, view your lab results or leave a message for your provider.
